Requirements / Forms

League Requirements

Skaters must:

~ be between 10 and 17 years of age.
~ have their own skates and gear
   (some rental skates are available)
~ carry USARS (USA Roller Sports) insurance    (approximately $35 a year)
~ pay monthly dues -$20 per month.
~ commit to regular attendance
   (Minimum 50% Regular and 50% of Scrimmages)

Equipment required for ALL practices:

1. Knee pads
2. Elbow pads
3. Wrist guards
4. Helmet
5. Mouth guard
6. 1 Black AND 1 White shirt (for scrimmages and drills)
7. WATER BOTTLE!!!

Some skates are available to rent from the league for $5.00 per month.

Forms:

Welcome Packet, Registration Form, and USARS form will all be available at the first day of practice, or can be obtained beforehand by sending an email request to info@emeraldcityjuniorgems.com.

Important note: USARS requires proof of birthdate (copy of birth certificate, driver's license, or passport) for all new members, please plan ahead to ensure you have this documentation prior to registration.
